Obtaining a Death, Birth, Marriage / Domestic Partnership Certificates

Requestors Must Be Able to Identify a Birth Record by Providing Exact Information

  • The exact name as it is currently recorded on the Birth Record (First Middle Last)
  • The exact place where the birth event occurred (City)
  • The exact date the birth event occurred (Month Day Year)
  • Parents' Names at Birth (When Recorded)

Multiple years will not be searched unless the event occurred 80 years (or more) ago.

Requestors Must Be Able to Identify a Death Record by Providing Exact Information

  • The exact name as it is currently recorded on the Death Record (First Middle Last)
  • The exact place where the death event occurred (City)
  • The exact date the death event occurred (Month Day Year)
  • Parents of the deceased subject's names at birth (If Recorded)

Multiple years will not be searched unless the event occurred 40 years (or more) ago.

Requestors Must Be Able to Identify a Marriage Record by Providing Exact Information

  • The exact names of Spouse A and Spouse B, at birth, as they have been recorded on the Marriage Record (First and Last)
  • The exact place where the marriage occurred (City)
  • The exact date the marriage event occurred (Month Day Year)

Multiple years will not be searched unless the event occurred 40 years (or more) ago.

Requestors Must Be Able to Identify a Civil Union Record by Providing Exact Information

  • The exact names of both Party A and Party B as they have been recorded on the Civil Union Record (First Last)
  • The exact place where the civil union occurred (City)
  • The exact date the civil union occurred (Month, Day, Year)

Requestors Must Be Able to Identify a Domestic Partnership Record by Providing Exact Information

  1. The exact names of both partners as recorded on the Domestic Partnership record
  2. The date the Domestic Partnership was registered

Documents will only be released to the subject or the subject's parent, spouse, child, grandchild, sibling or legal representative. Requestors other than the subject must provide proof of relationship to the subject.

All requestors must show id before a certificate will be issued:

Must provide photo driver's license with address or Photo non-driver's license with address issued by Motor Vehicle Agency

Or

Photo ID without address and One additional form of ID with address

Or

Two alternate forms of ID with an address such as a non-photo driver's license, vehicle registration, insurance card, voter registration, passport, green card, county ID, school ID, utility bill(s), or other as approved by the State Registrar

Certificate Costs

  • Birth, Marriage, Civil Union, or Domestic Partnership Certificates: $25 for the first certificate and $3 for each additional certificate
  • Death Certificates: $10 per certificate
  • Certifications of Birth, Death, Marriage or Civil Union: $10 per certification
  • Corrections to Birth, Death, Marriage, or Civil Union Certificate: $10 per correction
  • Search of vital records: $3 per year searched
  • Please make checks or money orders payable to the "Borough of Point Pleasant Beach"
